Dr. Wayne
Case, MD
Wayne Case is a rare South Florida native who was born, raised, and educated in Dade County, Florida. He is married and has three grown children. He graduated from the University of Miami School of Medicine in 1971 and proceeded to do his residency at Jackson Memorial Hospital. He is board certified by the American Board of Physician Specialists and also the American Board of Quality Assurance. His practice first opened in 1971 and has been open continuously since.

Dr. Cheryl
Case-Diaz, MD
Cheryl Case-Diaz was born and raised in our South Florida community. She is currently married and has two children. She attended undergraduate school at Vanderbilt University. She then trained at the University of Miami for medical school where she graduated in the top 10% of her class. She continued on to her residency at Jackson Memorial Hospital in Internal Medicine and finished in 2004 . She is board certified by the American Board of Internal Medicine. She joined the Family Medical Centre in 2004.

Dr. Olga
Perez, MD
Olga Perez, M.D. was born in Cuba. She attended medical school at Universidad Central del Este in the Dominican Republic. She then continued her residency training in Family Medicine at Tulane Medical School/East Jefferson General Hospital in New Orleans, Louisiana. She is board certified by the American Academy of Family Medicine. She holds the position of Voluntary Faculty at the University of Miami School of Medicine. She is currently married with 2 daughters. She joined our practice in 2014.

Michael
Paritzky, PA
Prior to joining the Family Medical Centre as one of our physician assistants, Michael J. Paritzky was a podiatric surgeon having practiced for 20 years in the Miami Lakes–Hialeah area of Miami. Born and raised in Baltimore, Maryland, Michael graduated with a BA degree from Yeshiva University in New York having majored in pre-med. He then went to the California College of Podiatric Medicine in San Francisco, California where after four years of schooling he received his Doctorate of Podiatric Medicine. He remained in the Bay Area completing a two-year vigorous surgical residency in reconstructive surgery of the foot and ankle. He came to Miami Lakes in 1980 where he established his podiatric practice. Board certified in foot and ankle surgery by the American Board of Podiatric Surgeons, he was asked to chair the department of podiatric surgery at Palmetto General Hospital in 1987, a position he held until 1999. His desire to learn and expand his horizons led him to return to school to become a physician assistant. After returning to school full time for three years, Michael graduated from the Miami Dade Physician Assistant Medical Program in July of 2011. Michael is married to his better half Margaret for 37 years and has four wonderful married children who have blessed them with 15 beautiful grandchildren

Yenma Vidal Carmona, ARNP
Yenma Vidal Carmona was born in Cuba and moved to the United States in 2016. She graduated as a Medical Doctor in 2013 and worked as a family doctor in Cuba for 3 years. When she arrived in this country she began to study and improve herself to be able to practice her profession. She became a Registered Nurse and worked in that profession for almost 2 years while she continued her studies until she graduated as a Nurse Practitioner. She loves her career, and her priority is the health and well being of her patients. In her free time she loves to travel and spend time with her family. Throughout her entire life, especially this time as a Nurse Practitioner, she has learned to become a well-rounded individual with not only having the medical knowledge but also the compassion and love to treat patients, to educate them and their families and to dedicate her life to her career.

Lianet Suarez, ARNP
Lianet was born and raised in Cuba. She moved to the US in 2011 where she started her career as a nurse in Tulsa Community College (TCC). She then moved to Miami and graduated with a Nursing Associate in 2015 at Dade Medical College and she started working at Mount Sinai Medical Center as a nurse. As a nurse she worked in many units like ICU’s, Neuro, Ortho, PCU’s and Transplant unit at MSMC and Cleveland Clinic. She completed her Bachelors in Nursing Degree at Chamberlain University in 2018. After working a few years as a nurse, she realized her passion has always been to become a primary care provider. She decided to go back to school and start the Master Degree in Nursing. She graduated as a Family Nurse Practitioner in 2021 at Miami Regional University (MRU) and joined Family Medical Centre in February, 2023. She loves to travel and enjoy time with her family. Her happy place is the beach.

Yanet Gonzalez Casanova, FNP-BC, ACNP-BC
Yanet Gonzalez Casanova is a dedicated and compassionate Nurse Practitioner with a rich background in patient care and clinical excellence. Born and raised in Cuba, she moved to the United States in 2008 and began her healthcare career as a medical assistant in 2009. Her passion for nursing led her to pursue formal training, and she graduated as a Registered Nurse in 2018.
Yanet joined Mount Sinai Medical Center, where she gained diverse experience across Cardiac, Neurology, Orthopedics, and Medical-Surgical units. Her years at the bedside deepened her commitment to holistic care and inspired her to advance her role as a provider. She earned her Family Nurse Practitioner certification in 2021, followed by her Acute Care Nurse Practitioner certification in 2024.
Currently, Yanet practices at Family Medical Centre, where she integrates her broad clinical background with a patient-centered approach to primary care. She is actively preparing to pursue her Doctorate in Nursing Practice (DNP), driven by a lifelong commitment to learning and leadership in healthcare.
Outside of medicine, Yanet finds joy in traveling, spending time with her family, and tending to her garden—her personal sanctuary and source of peace.

Shon Samuel, APRN
Born and raised in New York City, Shon traded snowy blizzards for hurricane winds when he relocated to pursue his passion for healthcare in warmer climates. Shon is proud to be married to his beautiful wife and is the beloved “fun uncle” to twelve energetic nieces and nephews.
Shon began his academic journey at the College of Staten Island (CUNY), where he earned his first Bachelor's degree in Sociology/Anthropology. Driven by a desire to make a direct impact in healthcare, he pursued a second Bachelor's of Science in Nursing at Chamberlain University, followed by a Master of Science in Nursing at Walden University, graduating Magna Cum Laude as a Family Nurse Practitioner.
With over five years of hands-on healthcare experience, Shon has contributed to top medical institutions including the University of Miami, Memorial Hospital, and the Cleveland Clinic. His clinical rotations at Cleveland Clinic Weston further enriched his training and deepened his commitment to patient-centered care.
Now practicing at Family Medical Centre, Shon treats his patients like family. As a certified Family Nurse Practitioner, he is dedicated to delivering compassionate, evidence-based, and personalized care to every individual he serves.

Siukei Cordero, APRN
Siukei Cordero is a compassionate Nurse Practitioner who earned both her Bachelor of Science in Nursing and Master's degree from West Coast University, graduating with honors. She dedicated most of her nursing career to serving on the front lines during the COVID-19 pandemic, an experience that deepened her commitment to patient-centered and preventive care. While working in inpatient care at Memorial, Siukei developed a strong passion for family medicine after recognizing the vital importance of prevention in improving long-term health outcomes. Outside of her professional life, Siukei treasures her role as a mother and enjoys traveling, providing her children with enriching life experiences. She finds peace by the ocean and frequently participates in beach cleanups to help preserve the environment. In her free time, she also enjoys crafting, community outreach, and high-intensity interval training (HIIT) as part of her wellness journey.
